Instructions
In this study, you will be asked to imagine yourself in a state of severe dementia, and then consider a series of separate, potential medical decisions. These situations are complex, and it is important to consider all of the details and outcomes associated with them. So, please take your time to read the descriptions carefully. You may take as much time as you need to review them.

First, you will read about your medical history and overall quality of life in a state of severe dementia. You will then consider a health crisis that may arise during this state of severe dementia and its corresponding medical treatment decision. You will then be asked about what you would prefer to be done in that scenario. When you read about each scenario, assume that you have not experienced any of the previous scenarios.

Keep in mind that there is no correct answer to any of these treatment decisions. Individuals may respond differently based on their religious and moral convictions, family situations, and other considerations. We ask that you respond based on your own personal preferences.

Although it may be hard to think about, please try your best to imagine yourself in these situations and answer each question honestly. We recognize this may be uncomfortable for some participants. You are free to end your participation by clicking out of the survey at any time.
